For (4), I can think of a few that I've, umm, used in vids myself :-). In A Little Less Conversation, there's a shot from Highlander of Methos unwrapping a sword and offering it to Duncan - it's kind of a "classic" HL image in vids, and it's from the episode Deliverance. Also in Conversation, there's a shot from ST III: The Search for Spock, where a large phallic rock erupts out of the planet towards the end of the film.
Also, in the episode You're Welcome from Angel, there's kind of a hilarious moment where Lindsay's tiny dagger turns into a huge sword - I used it in Don't Cha.
Oh, a couple that I haven't used and that are unquestionably intentional - the Disney movie Mulan. At the end, the big battle between the evil guy and Mulan. The hero initially attacks the big evil guy, but he's carrying only a teeny dagger against evil guy's big sword, and he's defeated. Then evil guy comes after Mulan with his big sword - and she snaps her circular fan around it and yanks it away. It's freaking amazing.
Also, earlier in the film, there's a scene where Mulan wins a contest where the soldiers have to climb a large pole with two swinging circular weights strapped to their arms. The men keep climbing with the weights dangling, and fall. Mulan ties the weights in a knot and uses them as a sort of lever to pull herself up. Again, no doubt this was deliberate.
